United Way Payroll Deduction Pledge Form

Please fill in your personal information and choose one distribution option (a UWGMC fund, program, or non-profit agency of your choice). If no distribution option is chosen, your donation will go to the UWGMC Community Impact Fund. Give directly to United Way of Greater Mercer County programs.
Minimum donation of $52 is required. A nine percent processing fee not to exceed $250 will be deducted. Nonprofit agency must have 501(c)(3) tax status and comply with the Patriot Act.
Please list the name(s) of the organization(s). If listing multiple agencies, please include how much each should receive. The non-profit agency must be a valid nonprofit with IRS 501(c)(3) tax status and in compliance with USA Patriot Act requirements to be eligible. A minimum donation of $52 is required and a 9 percent processing fee, not to exceed $250, will be deducted.
Total donation amount for the entire year.
